지방재정 예산지원을 고려한 COVID-19 확진 발생 결정요인 분석

본 연구는 2020년4월1일~2020년12월16일 시도별 사회적거리두기 대응, 의료 및 일자리 재정지원, 지역 및 권역별 차이, 인구학적 및 종교요인을 고려한 지역특성을 반영하여 광역시도별 COVID-19 확진자 발생에 대한 결정요인을 생존모형을 응용하여 파악하는 것을 연구의 목적으로 한다. Kaplan- Meier 분석 결과, 시도 및 권역별로 COVID-19 확진자 발생은 차별적으로 이루어진 것으로 나타났으며, 이를 통해 COVID-19 확진 확률 역시 일정한 차이가 있는 것을 확인하였다. 또한, Cox 비례위험 모형을 활용하기 위한 5개 부문, 12개 변수에 대한 비례위험 가정 결과 12개 변수는 위험곡선이 교차하지 않고 관측값과 기댓값 분포가 동일하여 Cox 비례위험 가정을 충족하는 것으로 분석되었다.
따라서, Cox 비례위험 모형을 활용하여 COVID-19 확진 발생의 결정요인을 도출한 결과, 사회적거리두기 단계가 낮고, 고령 및 평균연령이 높으며, 상대적으로 의료진이 적으며, 의료시설 투자 및 보건의료복지 지방재정 지원력이 낮은 비수도권 지역, 재정자립도가 낮은 지역, 낮은 간호사 수, 의료시설이 상대적으로 적은 지역에서 COVID-19 확진 위험이 높은 것으로 분석되었다. 더불어, 12개 변수에 대한 Cox 비례위험함수를 사용한 가설 검정 결과에서도 동일한 결과가 도출됨으로써 COVID-19 확산 위험에 대한 확인과 함께 변수별로 지역적인 차이가 있는 점이 확인되었다.

An empirical analysis of determinants of COVID-19 considering local fiscal budget support

The purpose of this study is to estimate empirically the determinants of the occurrence of COVID-19 with considering social distance correspondence, medical care and jobs, financial support, local and regional differences, demographic and the religious factor from April 1st 2020 to December 16th 2020. As a result of Kaplan-Meier analysis, it was found that the occurrence of COVID-19 by metropolitan municipal regions was differentiated, and through this, it was confirmed that there is a certain difference in the probability of COVID-19 confirmation. In addition, as a result of the analysis for testing the proportional hazard assumption of 5 sectors and 12 variables for utilizing the Cox proportional hazard model, the 12 variables do not cross the risk curve and show the same distribution of the observed and expected values, which satisfies the Cox proportional hazard assumption.
Therefore, as a result of deriving the determinants of COVID-19 using the Cox proportional hazard model, low the social distancing stage, high number of ration of the elderly and the average age, relatively few medical staff and medical facility, low local financial support for medical facilities and jobs in non-metropolitan area, and low financial independence, relatively small number of nurses and medical facilities in area were analyzed to be at high risk of COVID-19 occurrence. In addition, the same results were obtained from the hypothesis test results using the Cox proportional hazard function for 12 variables, which confirming the risk of spreading COVID-19 and confirming that there are regional differences for each variable.
